Thank you for choosing Phase II Sexual Medicine and Menopause Center , Patricia Bearnson, M.D as your healthcare provider.  We are committed to excellent patient care. We look forward to meeting you and developing a long lasting relationship.  Our goal is to provide you with information that will answer your questions and help to make your first visit a pleasant one.

APPOINTMENTS

If for any reason you need to change or cancel an appointment, please do so as soon as possible.  Appointments cancelled or changed or patients that don't show for their appointment may incur a  $50.00 cancellation/change fee.

Because Dr. Bearnson is very thorough in the complex treatment of sexual health, hormone therapy and osteoporosis consults most first appointments are scheduled for 45 minutes. These appointments are not combined with wellness checks, annuals, Pap smears or other gynecological visits

Routine wellness visits, annuals, Pap smears and general gynecological needs are scheduled with our Nurse Practitioner, Ronda Lucey.  She sees patients on Fridays and Tuesdays.

INSURANCE

Prior to your appointment please check with your insurance company and make sure you are covered for the requested appointment or test.

Our agreement with your insurance company dictates that we collect your co-payment at the time of your visit.  We are unable to bill your co-payment, therefore we will need to reschedule your appointment if you come unprepared to make payment.  

As a courtesy our billing office will bill your insurance company.  We will bill all insurances however if we are not a provider for your insurance company we will ask you to pay for the visit at the time of your appointment.  Please make sure you bring a current insurance card to every appointment.

PAYMENT

If you are a self-pay patient we require that you pay at the time of your appointment.  Patients with insurance are required to pay their co-pay at the time of the visit.  Those unprepared to pay at the time of the visit will be billed a $25 service fee.

• Arbitration - Arbitration is a method of resolving issues or claims without going through the court system.  When a problem arises, we first want you to come to us to address it.  Most concerns or claims have historically been resolved by simply working together to find a reasonable outcome.  Dr. Bearnson encourages you to sign this document.
