APPOINTMENTS
If for any reason you need to change or cancel an appointment, please do so as soon as possible. Appointments cancelled or changed or patients that don't show for their appointment may incur a $50.00 cancellation/change fee.
Because Dr. Bearnson is very thorough in the complex treatment of sexual health, hormone therapy and osteoporosis consults most first appointments are scheduled for 45 minutes. These appointments are not combined with wellness checks, annuals, Pap smears or other gynecological vi sits.

Routine wellness visits, annuals, Pap smears and general gynecological needs are scheduled with our Nurse Practitioner, Ronda Lucey. She sees patients on Fridays and the first Thursday evening of every month.

NOTICE OF PRIVACY POLICY
At Phase II Sexual Health and Menopause Center we are committed to protecting your privacy. If you would like to review our complete privacy policy please contact our office for a copy of that document.
PAYMENT
If you are a self-pay patient we require that you pay at the time of your appointment. Patients with insurance are required to pay their co-pay at the time of the visit. Those unprepared to pay at the time of the visit will be billed a $25 service fee.
We accept Visa, Mastercard and American Express.
INSURANCE
Prior to your appointment please check with your insurance company and make sure you are covered for the requested appointment or test.
Our agreement with your insurance company dictates that we collect your co-payment at the time of your visit. We are unable to bill your co-payment, therefore we will need to reschedule your appointment if you come unprepared to make payment.
As a courtesy our billing office will bill your insurance company. We will bill all insurances however if we are not a provider for your insurance company we will ask you to pay for the visit at the time of your appointment. Please make sure you bring a current insurance card to every appointment.
