APPOINTMENTS

If for any reason you need to change or cancel an appointment, please do so as soon as possible.  Appointments cancelled with less than 24 hours notice, or failure to show for their appointment may incur a  $50.00 cancellation/change fee.

Routine wellness visits, Pap smears or general gynecological needs are typically scheduled with our Nurse Practitioner, Ronda Lucey.  She sees patients on Fridays and the first Thursday evening of every month.

Be sure to visit t Phase II SkinCare to discover the other services available at Dr. Bearnson's.  Come in for an annual and treat yourself to one of our exciting new skin treatments. Our newest treatment available treats unwanted cellulite.  Through a combination of laser, radio frequency, and suction VelaSmooth can accomplish amazing results! There are many lasers on the market; their effectiveness vary greatly.  Dr. Bearnson selected the top of the line equipment to assure effective, safe, and satisfactory results.

PAYMENT

If you are a self-pay patient we require that you pay at the time of your appointment.  Patients with insurance are required to pay their co-pay at the time of the visit.  Those unprepared to pay at the time of the visit will be billed a $25 charge fee.

INSURANCE

Prior to your appointment please check with your insurance company and make sure you are covered for the requested appointment or test. 

As a courtesy our billing office will bill your insurance company.  We will bill all insurances however if we are not a provider for your insurance company we will ask you to pay for the visit at the time of your appointment.  Please make sure you bring a current insurance card to every appointment.

BILLING INFORMATION

Our billing is handled by an outside billing firm.  Questions regarding your bill may be directed to 801.253.4103. 

RECORDS REQUEST

We will provide copies of your medical records upon request after the completion of a REQUEST FOR MEDICAL RECORDS RELEASE is completed by you and received in our office.  Records request may require a $50 fee. There is an additional fee of $15 for same day requests. 

 

DIRECTIONS

We are located between 1300 East and Highland Drive on the south side of

4500 South, an Arts and Crafts style, yellow stucco building .   Look for the

green SILVARADO Senior Living sign.

• I15 take the 4500 South exit and go east
• South bound I215 belt route take the 4500 South exit and go west
• East bound I215 exit 2000 East.  Travel North on 20th East.  20th East splits at 6200 South, veer to your right to VanWinkle.  At 1300 East turn right .  At 4500 South turn right.
